Medical Record Self-Audit

This form should be used as part of a practice internal audit as discussed in Chapter 3–Compliance of a specialty-specific Reimbursement Guide or the ChiroCode DeskBook. It simplifies the process of chart reviews.

Some practices use a form like this on every chart before it is filed. This is only a sample – individual providers may need to make modifications for specific state or payer requirements.

If errors are discovered during your internal audit, address them immediately and document your corrections properly in your compliance log.
