24-Hour urine calcium test

The physician orders a 24-hour urine calcium and we assigned CPT code 82340, Calcium; urine quantitative, timed specimen. However, at some of our labs, a 24-hour urine calcium test always include a urine creatinine result as well. Because of this, some of our coding professionals feel we should report both CPT code 82340 for the 24-hour urine calcium and CPT code 82570, Creatinine; other source, for the urine creatinine. Our question is whether it would be appropriate to report both CPT codes when results show both a 24-hour urine calcium and creatinine, but the physician order does not mention creatinine? ...
