Correction: CPT code assignment based on a diagnosis

The coding advice published in Coding Clinic for HCPCS, First Quarter 2014, Page 2, regarding the statement that, “Code assignment is never based on the diagnosis that is listed as the reason for the procedure”, requires correction.
