Defibrillation threshold testing

A patient is seen for implantation of a single chamber implantable cardioverter-defibrillator (ICD). Based on documentation the defibrillation threshold testing (DFT) was not performed because the risks outweighed the benefits. Therefore, for this case would it be appropriate to assign CPT code 93641 for the procedure performed? Would the defibrillation threshold evaluation/testing need to be performed in order to assign CPT code 93641 or should a modifier be appended to the CPT code because the defibrillation threshold testing was not performed? ...
