Fecal transplant via colonoscopy

What would be the appropriate CPT code(s) for fecal transplant via colonoscopy for Medicare and non-Medicare patients? I have seen conflicting information. Some information says to use 44705, Preparation of fecal microbiota for instillation, including assessment of donor specimen, and 45378, Colonoscopy, flexible, proximal to splenic flexure; diagnostic, with or without collection of specimen(s) by brushing or washing, with or without colon decompression (separate procedure), or code 45378 without code 44705, or CPT code 44799, Unlisted procedure, intestine, or HCPCS code G0455, Preparation with instillation of fecal microbiota by any method, including assessment of donor specimen. Also, if CPT code 44705 or HCPCS code G0455 are to be reported, does the physician need to document the preparation of fecal microbiota? Our physicians document a colonoscopy with infusion of fecal effluent. ...
