There are several HCPCS Level II J-codes for Lupron. HCPCS code J1950, Injection, leuprolide acetate (for depot suspension), per 3.75 mg, J9217, Leuprolide acetate (for depot suspension), per 7.5 mg, and J9218, Leuprolide acetate, per 1 mg. Unofficial references indicate how to code Lupron based on different diagnoses and/or treatments, however they appear to be inconsistent.
