Multiple laparoscopic hernia repairs

The right inguinal, femoral and obturator hernias are repaired laparoscopically, in the right groin area. The umbilical hernia is repaired via an open approach through one of the laparoscopic trocar incisions at the umbilicus. Can the laparoscopic inguinal, femoral and obturator hernia repairs and the open umbilical hernia repair all be coded, on the same encounter, taking into consideration the NCCI guidelines? Is the laparoscopic inguinal hernia repair considered an “abdominal procedure” and therefore the other hernia repairs would only be reportable when performed at a site other than the incision, and if medically reasonable and necessary? Is the open umbilical incision considered the same site incision as the laparoscopic procedures? ...
