No-cost full credit and partial credit modifiers
The Centers for Medicare and Medicaid Services (CMS) no longer recognizes modifiers FB, Item provided without cost to provider, supplier or practitioner, or full credit received for replaced device (examples, but not limited to, covered under warranty, replaced due to defect, free samples), or FC, Partial credit received for replaced devices, to identify a device that was furnished without cost or with a full or partial credit. This change has been effective since January 1, 2014.
