Pulmonary vein isolation

Our facility is seeking clarification regarding the appropriate reporting of pulmonary vein isolation performed for the treatment of persistent atrial fibrillation via intracardiac catheter. For this procedure, patients are brought into our electrophysiology lab in a fasting state where general anesthesia is provided. 3D mapping is provided and radiofrequency ablation is performed. Circular ablations are then made around each of the four pulmonary veins and pulmonary vein isolation is successfully performed on all four pulmonary veins. A cardiac society has published guidance to report CPT code 93651 for the pulmonary vein isolation. However, some payor policies have suggested unlisted CPT code 93799.
