Reporting Of Procedure-To-Device Edits

In question 3 of the Third Quarter 2009 issue of Coding Clinic for HCPCS, it was advised that CPT code 77002, Fluoroscopic guidance for needle placement (g, biopsy, aspiration, injection, localization device), be reported for Lap Band Fill. However, for Medicare reporting, if we only report CPT code 77002 on the encounter an OCE 27 edit is generated stating only incidental services provided. Would it be appropriate to report CPT code 17999, Unlisted procedure, skin, mucous membrane and subcutaneous tissue, for the injection into the Lap Band Port in addition to 77002? If not, could we report CPT code 96372, Therapeutic, prophylactic, or diagnostic injection (specify substance or drug); subcutaneous or intramuscular, for the gastric band adjustment? Please advise us on the appropriate reporting of Lap Band Fill. ...
