Sclerotherapy of venous malformation

A patient presented for percutaneous sclerotherapy for a left lower extremity (LLE) venous malformation. Following general anesthesia administration and under direct ultrasound guidance, the lesion was identified and accessed at 11 separate sites with blood return from each access site. Contrast was injected at each access site to confirm intralesional access. The lesion was then treated with three rounds using 3% sodium tetradecyl sulfate (STS) liquid plus contrast foam in a 1:1:1 mixture injected through each of the access cannulas followed by a 20-minute dwell time. Approximately halfway through the final dwell time, the access cannulae were slowly removed to facilitate thrombosis along the tracts.
