Shoulder hemiarthroplasty converted to total arthroplasty

A patient with a previous shoulder hemiarthroplasty with bipolar prosthesis presents for conversion to a reverse total shoulder arthroplasty (TSA). A skin incision was made over the anterolateral aspect of the left shoulder extending down to the deltoid insertion. The deltopectoral interval was dissected, thus releasing significant scar tissue. The pectoralis major was released off of the humerus allowing better exposure and the shoulder was dislocated. The humeral prosthesis was well fixed with no vertical instability and all soft tissue was removed. The bipolar head was extracted off the stem, revealing bone loss and nonunion fragments around the humeral calcar and of the lateral and proximal humerus due to the previous humerus fracture. The preparation of the humerus was continued by disrupting the cement mantle using reamers and confirming the patient’s bone was not compromised by using fluoroscopic guidance. A significant amount of scar and fibrous tissue was released, then a patient-specific guide was placed on the glenoid face. The central boss reamer was used to make a small boss site for the placement of the reconstruction system guide on the glenoid face. Placement of 5 screws was performed and there was good contact between the glenoid face and the posterior vault reconstruction implant system component. After placement of the screws for fixation, the glenosphere was impacted onto the glenoid baseplate. Once the trial humeral component was placed for proper fit and then removed, final reaming of the humerus was performed. Fibrous fragments were secured to the humeral shaft and the reverse prosthesis was coated in gentamicin impregnated bone cement and the stem was placed in retroversion. The shoulder was reduced after the polyethylene component was seated on the stem. Advice published in Coding Clinic for HCPCS , Third Quarter 2021 regarding a partial knee converted to a total knee advised the revision code was appropriate to capture the knee conversion.
