Steroid injection for Dupuytren’s contracture

A patient presented with a painful left hand. On examination, the provider documented thickened palmar fascia, fibromatosis and nodular tissue consistent with Dupuytren’s contractures of the proximal interphalangeal joint of the small finger and the metacarpophalangeal joint of the ring finger. Due to the patient’s tenderness over the nodular tissue at the base of the small finger, the tender area was prepped for a steroid injection. Next, 0.5 mL of 0.25% Bupivacaine and 0.5 mL of 40 mg/mL of triamcinolone were injected into the Dupuytren’s palmar nodules. CPT code 20527 is for injection of an enzyme into the palmar fascia, for treatment of a Dupuytren’s contracture.
