Our facility needs clarification regarding how to properly report HCPCS code range G0416–G0419 (surgical pathology, prostate) and CPT code 88305, Level IV — Surgical pathology, gross and microscopic examination. Should we follow the HCPCS long code descriptor or the NCCI Policy Manual guidelines? Also, the codes G0416–G0419 identify prostate needle saturation biopsy. Can these HCPCS G-codes be reported and billed regardless of the method utilized for the biopsy?  ...
