The Central Office on HCPCS frequently receives requests for assistance related to the correct coding for lesion removal. Some questions are directed at determining the correct CPT code when the documentation does not clearly indicate if a lesion was removed by excision, cutting off or cutting out tissue, or by destruction of tissue.
