The patient is a 60-year-old male status post right carotid endarterectomy 16 months ago, who presents to the hospital with recurrent right carotid artery stenosis. The patient was taken to the operating room and the surgeon performed a redo right carotid endarterectomy with replacement of the proximal internal carotid artery with interposition reversed saphenous vein graft from the right groin and vein patch angioplasty of the interposition vein graft. Would the redo of the endarterectomy be considered a revision of a vascular procedure (code 39.49)? If not, what is the most appropriate code(s) assignment for this procedure? ...
