Heart Transplantation due to Congestive Heart Failure, Dilated Cardiomyopathy, Mitral Valve Disorder and Rheumatic Heart Disease
The patient is a 49-year-old female, who was admitted for heart transplantation because of end-stage congestive heart failure due to dilated cardiomyopathy secondary to rheumatic heart disease with mitral valve compromise. The patient previously had mitral valve replacement with prosthetic valves X2 and implantation of automatic cardioverter/defibrillator. Gradually, the patient developed valvular cardiomyopathy and congestive heart failure. The surgeon carried out orthotopic heart transplantation and removal of automatic implantable cardioverter device (AICD) and leads. Would the congestive heart failure be coded as rheumatic heart failure and should the mitral valve compromise be coded as a complication of the previous valvular surgery?
