Coders should carefully review the entire operative report to determine the clinical significance of the adhesions and the complexity of the lysis of adhesions. Coders should not code adhesions and lysis thereof, based solely on mention of adhesions or lysis in an operative report. As is customary with other surgeries, it is irrelevant whether the adhesions or lysis of adhesions are included in the title of the operation.
