An 89-year-old woman presented to the Emergency Department (ED) due to an unwitnessed fall. The patient was diagnosed with a urinary tract infection and treated with Bactrim. During the ED encounter, the patient became progressively more somnolent. Additional workup revealed elevated troponin level and intermittent atrial fibrillation. The patient was admitted for further cardiology management. She never reported any chest pain; did not demonstrate electrocardiogram (ECG) changes; troponin levels stabilized; and at discharge, the provider diagnosed non-traumatic myocardial injury.
