Recurrent Renal Cell Carcinoma

A patient who is status post left nephrectomy presents with a history of renal cell carcinoma and a retroperitoneal mass is identified in the left kidney bed. The patient is admitted for surgical resection. The physician documents left recurrent renal cell carcinoma and the pathology report confirms this diagnosis. How is this diagnosis coded? Should a code for primary carcinoma of the kidney be assigned even though the kidney is no longer present? Would this be considered a primary carcinoma of the retroperitoneum or a metastasis to the retroperitoneum and history of renal cancer? The Coding Clinic is unclear on this subject. ...
