Unroofing of Anomalous Coronary Artery

A patient with an anomalous origin of the right coronary artery was discovered to have significant coronary artery disease and presented for coronary artery bypass graft (CABG) with unroofing of the intramural segment of the anomalous right coronary artery. Following completion of the CABG, attention was turned to the ascending aorta. The tissue surrounding the aortic root was dissected and the right coronary artery was identified as it emanated from the right sinus of Valsalva. The intramural segment was unroofed with scissors and the intima was approximated on the media and adventitia, improving the ostium of the right coronary artery.
