AMA Health Insurance Claim Form
The current AMA Health Insurance Claim Form (HCFA 1500) has undergone major revisions and will soon be replaced by a new version. The new forms will be accepted by Medicare beginning November 1, 1991.
A six month period where submission of either form is allowed will follow (through April 1992). After April 1992, only the new form will be accepted.
To prepare for the new form, we suggest the following steps be taken:
