Do allergy injection codes (95115-95117) include a routine pre-injection evaluation by a nurse and a routine post-injection monitoring by a nurse? What services performed by the nurse would support reporting code 99211 and appending modifier 25, Significant, Separately Identifiable Evaluation and Management Service by the Same Physician or Other Qualified Health Care Professional on the Same Day of the Procedure or Other Service, ie, services that are not included when codes 95115-95117 are reported?
