Code 76145 was established in the CPT 2021 code set to be reported when a medical physicist performs a patient-specific peak organ dose calculation following a radiology procedure that exceeds the facility's established threshold for radiation dose. For example, a medical facility has an existing policy for patient peak skin dose (PSD) calculations following high-dose computed tomography (CT)-guided procedures, which seems to align with the intent and use of the code. In CPT ® Changes 2021: An Insider's View, the clinical example for code 76145 references fluoroscopy guidance for the procedure. However, while the code descriptor for code 76145 does not specifically reference CT-guided procedures, it does not seem to exclude them. Is it appropriate to report code 76145 for patient PSD calculations using CT-guided procedures?
