In the Data Element column for 99203/99213, it states "Review of prior external note (s) from each unique source." If a family physician reviews prior external notes from a cardiologist and also reviews prior external notes from a radiologist, would the review of those two unique sources result in a combination of 2 to meet MDM level Low 99203/99213?
