The descriptors for comprehensive electrophysiology (EP) study codes 93653, 93654, and 93656 include the conditional "when performed" for pacing and recording requirements. If one or more of the components of a comprehensive EP study were not performed without reasons for not performing them in the documentation, would it be appropriate to report codes 93653, 93654, and 93656 without a modifier, or would it be appropriate to append modifier 52 to these codes to indicate that a comprehensive EP study was not completed and the documentation elements were not met?
