A physician performed peripheral nerve blocks at the right dorsal ramus nerve levels L5, S1, S2, and S3, as follows: Using fluoroscopic guidance, a 25-gauge needle was advanced toward the junction of, and as close as possible to, each superior articulating process and transverse process or sacral ala for each median branch nerve indicated above. Once the needle position was optimized and verified, one cc of 1% lidocaine was injected over the medial branch nerve. Next, the needle New code 64451, Injection(s), anesthetic agent(s) and/ or steroid; nerves innervating the sacroiliac joint, with image guidance (ie, fluoroscopy or computed tomography), should be reported once for this procedure. The fluoroscopic guidance should not be separately reported as it is included in the work described with code 64451. was advanced toward the inferior lateral aspect of each sacral foramen. Once the inferior lateral aspect of the foramen was contacted, 1 cc of 1% lidocaine was injected. The sacral nerves were blocked as indicated above. Is it appropriate to report code 64450 for the injection procedure described?
