Regarding CPT codes 93293-93295 (REVISED IN 2015): is there any scenario that allows these codes to be billed more than 1x per 90 days (for example, the patient is experiencing a new symptom (e.g., syncope) or a misfiring of the device) ? Or are these codes reported only 1x regardless of the medical necessity of multiple evaluations within the 90 day period.
