Services were reported by a physician for a patient encounter at the hospital emergency room. On the same date, the patient was admitted to a nursing facility. The initial nursing facility care services were completed by a consultant. What is the appropriate code the consultant may report for their initial services at the nursing facility? Would modifier -25 apply?
