2017 CPT update: Physical therapy evaluation codes will require detailed documentation

Practices that provide physical therapy should take note of pending changes to therapy evaluation coding. According to the proposed 2017 Medicare physician fee schedule, CPT intends to delete the existing therapy evaluation and re-evaluation codes and replace them with new codes based on the complexity level.
