Getting paid E/M codes with modifier 25 face drastic pay reduction for some practices

Watch your E/M claims when appended with modifier 25 (Significant, separately identifiable E/M service). At least one Medicare Advantage carrier that operates in 25 states started slicing payments for those E/M claims in half on Aug. 1.
