High denials for modifier 25 span frequently billed E/M services

Be careful when you bill an E/M service on the same day as a separate service or procedure, as practices get tripped up when they use modifier 25 (Significant, separately identifiable E/M service by the same physician on the same day of the procedure or other service) to report a separate event.
