Physician TipSheet: Quality reporting — Falls risk assessment and plan of care measures checklists, July 2015
Use the following checklists, based on APCPS analysis of the 2015 PQRS measure specifications manual for individual measures, to report the two physician quality reporting system measures for falls. Measure #154 – falls risk assessment – may be reported for every patient age 65 or older in all non-acute settings. The provider should perform and report measure #155 – falls plan of care – if she documents that the patient is at risk for falls.
