Question: Our provider performed a total of four unilateral lumbar paravertebral facet radiofrequency ablations (RFA) during four different sessions. We received a policy limitation denial for the third and fourth procedures even though the current local coverage determination (LCD) allows four therapeutic procedures in a rolling 12-month period.
