Question: Encounter notes indicate that the ED provider performed a digital block on two levels of a patient’s intercostal nerve. Should I report 64420 x 2?

Answer: No, you should report:
64420 (Injection(s), anesthetic agent(s) and/ or steroid; intercostal nerve, single level) for the first-level injection
+64421 (… intercostal nerve, each additional level (List separately in addition to code for primary procedure)) for the second-level injection
Remember: Only use +64421 with 64420 — not with any of the other codes for anesthetic agent/steroid injections.
