Question: I need a clarification on professional fees POS 24. Procedure 64483 was performed in an ASC. Can I bill 77003 along with modifier 26?  Kentucky Subscriber Answer: The injection code 64483 (Injection[s], anesthetic agent and/or steroid, transforaminal epidural, with imaging guidance [fluoroscopy or CT]; lumbar or sacral, single level) includes the use of image guidance in the code description.
