It is intended to capture the risk factors identified by the Joint committee on Infant Hearing (JCIH) in appendix 2 of their recommendations (Pediatrics 2007;120;898-921). The user should always provide an answer (none if no risk factors exist) and otherwise pick as many answers as apply. The observations must be included IF the hearing screening result is delivered as part of this report.
