description | A request by a pharmacist (dispenser) for a formal prescription from a prescriber (e.g. physician). The prescription requests legitimatizes the dispensing of therapeutic goods following an instruction (e.g. through phone conversation or standing written agreement) from a prescriber. It has essentially the same contents as a prescription document except the pharmacist fills it out as a template for the doctor to sign. Information provided in the document includes patient identifiers (patient ID, name, address, etc.), pharmacy information, prescriber instruction details (prescription details, mode of communication), the prescription item and formula/dose information, and any notes from the pharmacist. This code was requested by, but not limited in use to, the Australian National E-Health Transition Authority Limited (NEHTA). The Prescription Request document is an officially recognized document in Australia and is used for legal purposes to allow the doctor to prescribe over the phone (or by another mode of communication) and fulfill the legal requirement to sign their request. | ||
