description | A brief medication treatment plan (MTP) is contained within the extended MTP document [LOINC: 77603-9] and contains one or more medication treatment plan items for a patient. The brief MTP describes the medications a patient is taking, will take or has taken as part of a plan. This term was originally created for, though not limited in use to, the IHE MTP profile in the Community Medication Prescription and Dispense (CMPD) set. The MTP profile states MTP items within the MTP section are very close to prescription (PRE) items but differ primarily in the position of the global workflow. While an MTP item is more an information piece (i.e. "this medication is part of the medication treatment plan"), a PRE item represents an instantiated prescription of the same medication. Most of the normative sections of the MTP profile will therefore be identical to the corresponding ones in the PRE profile. | ||
