description | The Chronic Respiratory Questionnaire Self-Administered, Individualized Format (CRQ-SAI) is derived from the original CRQ tool and used to measure health-related quality of life in patients with chronic obstructive pulmonary disease (COPD), chronic airflow limitation, or various lung disease. The CRQ-SAI contains individualized dyspnea questions in which respondents select the five most important activities that have caused them to experience shortness of breath during the past two weeks. Respondents are then asked to answer questions about their shortness of breath experienced when they have completed those chosen activities. Answers are scored on a 7-point Likert-type scale.
