descriptionA portable medical order form, commonly called a Physician Orders for Scope of Treatment (POST), POLST (Physician Orders for Life-Sustaining Treatment), or Medical Orders for Life Sustaining Treatment (MOLST), is a provider's order based on the patient's current medical condition and preferences. The form typically includes medical orders regarding CPR-code or no code status, the level of medical intervention (comfort measures, limited additional interventions, or full intervention), the preference for use of antibiotics (for comfort only or full treatment), and the use of medically administered nutrition. A protable medical order form may be changed or revoked by the patient or their authorized healthcare decision-maker (i.e. patient advocate or court-appointed guardian) at any time.
