As of 2019, CYP2C9, VKORC1, CYP4F2, and rs12777823 genotypes are used when available to help determine a patient's initial therapeutic warfarin dose. VKORC1 and CYP2C9 genotypes are the most important genetic determinants of warfarin dosing. Additionally, a genetic variation in the CYP4F2 gene, CYP4F2*3, as well as a variant near the CYP2C gene cluster, rs12777823, have been associated with influencing warfarin therapy.[NCBI Books: NBK84174] This panel is based on, but not limited in use to, the 2017 Update of the Clinical Pharmacogenetics Implementation Consortium (CPIC) Guideline for Pharmacogenetics-Guided Warfarin Dosing, which provides warfarin dosing recommendations for adults with and without African ancestry, and also for pediatric patients.[https://cpicpgx.org/content/guideline/publication/warfarin/2017/warfarin.pdf] | ||
